SMAS Facelift – Deep Plane Lifting
It is the most advanced facelift method. In the case of the classic facelift, an external pull was made on the skin, often leading to an unnaturally sagging result. With the SMAS Facelift we intervene in the deeper tissues, which are responsible for the youthful appearance of the face. This is the reason for the name of the technique, Deep Plane Lifting. It has been proven that in order to have a successful result, it is not enough to stretch the skin and remove its excess, since it does not offer support but coverage. Intervening in the SMAS, that is, in the network of connective tissue that is located under the skin and subcutaneous tissue (extending from the scalp to the neck), it constitutes one of the most important supporting structures of the face and neck. The doctor makes an incision just in front of the ear to access the SMAS and lifts it diagonally back and up, lifting it to the same position it was in 15 years ago. All the tissues in the middle and lower third of the face are automatically lifted, restoring its oval shape.

The mini-facelift is usually performed on younger individuals who have mild or moderate sagging. The procedure is performed under local anesthesia. With the mini facelift we can effectively treat the first signs of aging on the face (middle and lower part).
The doctor makes a small incision that extends from the top of the ear (in the hair) to below the lobe (it stops there, without going behind the ear as in the complete facelift) and approaches the SMAS and with special sutures, he lifts it to the position it was in previous years.
The MACS (Minimal Access Cranial Suspension) technique allows us to improve the appearance of wrinkles in the middle and lower part of the face, as well as restore the contour and lost volume in the cheeks. At the same time, we can smooth out wrinkles and sagging in the neck area.
Neck lift
Because the signs of aging do not stop at the face, a face lift is often combined with a neck lift. In this case, we extend the face lift incision behind the ear and into the occipital hairline, so that we can also lift the sagging tissues of the neck. When the neck sagging is mild, a neck lift can also be performed with a smaller incision under the chin.
Our goal is to address the image of the “sagging” neck. By strengthening the muscular plate, which is the most important muscle in the area with special sutures, we can restore the lost firmness, but also address the vertical strings-bands of the neck. If necessary, neck liposuction can also be performed to remove the double chin.
Forehead lift
When signs of aging on the forehead are severe, then we apply an endoscopic forehead lift. The coronal incision around the forehead is no longer necessary (except in rare cases) and 3-5 small incisions are sufficient, which are made inside the scalp, so that they are not visible. In this way, we achieve lifting of the tissues of the forehead and eyebrows.
Using an endoscope, the doctor intervenes specifically on the mimetic muscles that cause horizontal wrinkles on the forehead and vertical wrinkles between the eyebrows, due to expressiveness.
At the same time, we lift the tissues on the sides of the forehead area where the skin shows more pronounced relaxation, we raise the eyebrows and, if desired, we also lift the outer corner of the eyes that is sagging due to the weight of the drooping eyelids. After the operation, you will not need to do any additional intervention (e.g. botulinum toxin) for several years and usually you will not need to proceed with upper blepharoplasty either.
Combination of facelift and blepharoplasty
In the case that we want to deal with sagging and excess skin in the upper and lower eyelids, then we can combine the facelift with blepharoplasty. With this specific operation we can remove the skin that weighs down the look and makes the face look sad and tired. At the same time, we improve the problem of dark circles and eliminate bags under the eyes. The operation is short and is performed at the same time as the facelift. In the upper blepharoplasty, the incision is made exactly on the natural crease of the upper eyelid, while in the lower blepharoplasty the incision can be made either on the inside of the lower eyelid, or on the eyelash line.
After the surgery, the gaze appears more vibrant, while the peripheral field of vision, which in some cases is obstructed due to drooping eyelids, also improves.
Combination of facelift and liposuction
A facelift is a procedure that can redistribute facial volume, returning it to the positions it was in years ago. In cases where there is significant volume loss in the cheekbones or cheeks, a facelift is performed in combination with advanced liposuction techniques. We take fat from areas of the body with localized fat and, after special processing, inject it into the areas that need augmentation. The areas that are most often augmented are the temples, the area under the eyes, the cheeks and the chin.
It should be noted that autologous fat is completely safe because it comes from the body itself. From fat we can also extract pluripotent stem cells that enhance tissue regeneration and offer long-term results.

Plastic Surgeon George Velimvasakis leads the Antiagers Medical Team. He has more than 18 years of experience at an international level, across the full spectrum of plastic, reconstructive and aesthetic procedures. In his career, he has performed over 3000 procedures as a primary surgeon in both Greece and Germany. He holds dual certification from the Greek and European Committees of Plastic, Reconstructive and Aesthetic Surgery since 2018, as well as from the International Society of Hair Restoration Surgery (ISHRS).
He was born in Athens in 1974. In 2001 he graduated with "Excellent" (4.88/5) from the Medical School of Semmelweis University in Budapest. This is one of the top medical schools in Europe. He has received a Greek license to practice medicine and worked as a resident physician for 2 years in General Surgery at the Specialized Cancer Hospital of Piraeus "Metaxa". He then worked as a resident physician in Plastic Surgery at the Department of Plastic Surgery and Burns Unit "KAT".
He has been trained in the most modern techniques of plastic surgery on the face, neck, body and chest, through his participation in dozens of medical seminars, workshops, webinars and masterclasses.
Wanting to acquire a high-level specialty, he moved to Germany, where he obtained the German license to practice medicine after passing exams. He then worked in leading centers of Plastic and Aesthetic Surgery in German metropolises (Dortmund, Essen, Wiesbaden), which helped him enrich his knowledge and perfect his technique alongside renowned German scientists. Frequently Asked Questions
Will I need to stay in bed after the surgery?
No. Recovery is not painful. There may be some discomfort for the first 24 hours, but it is not enough to keep you in bed. You will need to limit strenuous physical activities for 15 days.
When will the stitches be removed?
The stitches placed in the front of the ear are removed after 5-6 days, while the stitches behind the ear are removed after 15 days.
When will I be able to return to work?
In about 15 days.
Will I need to wear a face mask?
Yes. You will be given a special elastic face-neck bandage to wear for 3 weeks. For the first two weeks, it is best to wear it 24 hours a day, while for the third week, 12 hours a day or night is sufficient.
Is the facelift scar visible?
With the modern SMAS Facelift technique, there is excellent healing of the incisions due to the lack of skin tension. That is, because we do not pull the skin to achieve tissue lifting but rather lift the deeper layers, there is not much force exerted on the scar site, so there is a risk that it will not heal well. In addition, a large part of the incision is well hidden within the scalp.
